6:00 p.m. Mandatory Parent Meeting/Turn in Forms

6:30 p.m. Player Photo’s

Be sure to bring:   

• $700.00 player dues payable to LCALB and will be cashed 6/5/2018 unless other arrangements have been made with the treasurer.
• List of any prescription medication you may be taking together with the name of the doctor who prescribed them.
• Valid photo identification for parent and player.  (Driver’s license, ID card or a school activity card will work for a player without a driver’s license)
• Insurance information and last 4 of players social security number to complete the Player Indemnification Agreement
• Calendar for noting down your committee times
• Original birth certificate for player
• Player and parent contact information

Player photos will be taken for the Tribune insert

Paperwork and Committee signups need to be completed
